1. What is the Siliq Solutions™ Instant Savings Program?
The Siliq Solutions Instant Savings Program provides eligible patients, with commercial or private insurance, with support for their
out-of-pocket costs for SILIQ™ injection.
2. Does the program only apply to prescriptions for SILIQ?
Yes, the instant savings apply only to your out-of-pocket costs for your SILIQ prescriptions.
3. Is there a limit to how much I can save?
How much you can save depends on your private or commercial health plan coverage for SILIQ™. Patient out-of-pocket costs can vary. If you are eligible and have private or commercial health plan coverage for SILIQ™ you may pay only $5 per month, subject to a $20,000 maximum benefit for 12 months from your first eligible date of participation in the program. You may re-enroll in the program annually, and continue to receive a benefit so long as you remain eligible. Before your 12-month expiration date, you’ll receive information about re-enrollment if you are still eligible to participate.
4. Am I eligible to participate?
To be eligible, you must have private or commercial insurance. You must be receiving treatment with SILIQ for an approved indication. You are not eligible if you can receive reimbursement of prescriptions, in whole or in part, by any federal, state or other governmental programs, including but not limited to Medicare (including Medicare Advantage and Part A, B and D Plans), Medicaid, TRICARE, Veterans Administration or Department of Defense health coverage, CHAMPUS, the Puerto Rico Government Health Insurance Plan, or any other federal or state health care programs.
5. Can I participate if my private or commercial insurance does not provide coverage for SILIQ?
If your private or commercial insurance does not cover SILIQ or your coverage is delayed or denied, you may receive SILIQ at $25 per month until coverage is approved or up to 12 months from program initiation.
6. Is there a maximum limit on how much support the program will provide?
Yes, if you have commercial insurance coverage for SILIQ, and you are eligible, you may pay $5 per month for SILIQ, up to a maximum savings limit of $20,000 for 12 months from your first eligible date of program participation. You will be responsible for all amount above this limit.
7. How can I enroll in the Siliq Solutions™ Instant Savings Program?
It's easy to enroll and receive your card if you are eligible. You can visit SiliqSavings.com or call 855-RX-SILIQ (855-797-4547). You will receive your instant savings information immediately, and then receive your card by mail in 5 to 7 days.You’re eligible to receive savings on your commercial health insurance deductible, co-pay, and co-insurance costs for SILIQ, up to a $20,000 maximum benefit. Before your
12-month expiration date, you’ll receive information about re-enrollment if you are still eligible to participate.
8. Do I have to activate my card?
No, your card is active and ready to use as soon as your enrollment is complete. You will be provided with the information on the front of your card when you enroll, and can use the information right away for instant savings with a pharmacy that has been certified under the SILIQ REMS Program, even before your card arrives in the mail.
9. Do I have to provide a receipt from the pharmacy as proof of purchase?
No, the pharmacy will handle the payment of your out-of-pocket cost to your private or commercial insurance provider.
10. Do I need to be concerned about the amount of funds left on my card?
No, there aren't any funds on your card. The pharmacy uses the Rx BIN number on your card to process your instant savings. If you would like to determine your remaining benefit for the program year, you can call 855-RX-SILIQ (855-797-4547), Monday through Friday, 8:00 AM –
8:00 PM Eastern Time.
11. What should I do if I lose my card?
No problem, just call the Siliq Solutions Instant Savings Program at 844-357-2013 to request a new card. We'll verify your information and send you a replacement card in the mail in 5 to 7 days.
12. Why do you ask for my Social Security number when I enroll?
We need your Social Security number so we can confirm that you are not participating in a government-funded healthcare program. This is a condition of your eligibility. We only use your number to verify this information, and it will not be stored or used for any other purpose. If you choose not to provide your Social Security number, we will need a copy of your health insurance card in order to confirm your eligibility.
13. Why do you ask for my phone number when I enroll?
We use your phone number to verify your home address. Your phone number will not be used or shared for telemarketing purposes.
14. What should I do if the pharmacy doesn’t accept my savings card?
You may still be able to receive savings on your out-of-pocket costs. Have your pharmacy call the Siliq Solutions Instant Savings Program at 844-357-2013 to discuss other payment options.
